What is an Ostenil® Plus injection and how does it work?

Ostenil® Plus is a hyaluronic acid (HA) injection designed to supplement the natural joint lubricant, synovial fluid. At Sonorad, we place the injection precisely into the affected joint under live ultrasound guidance, so the gel sits where it can cushion, coat and lubricate the joint surfaces. Ostenil® Plus includes mannitol, a stabiliser that helps protect the HA chains, so the product can act for longer once inside the joint. This mannitol-stabilised formula is the key differentiator versus standard HA products.

Ostenil® Plus is often delivered as a single treatment, with the option to repeat if symptoms return and after clinical review. The mannitol-stabilised formulation was developed to extend action so fewer injections may be required compared with some older HA schedules, although individual response differs. Discomfort is usually minimal. Local anaesthetic can be used if necessary to make the procedure more comfortable.

The typical use is symptomatic osteoarthritis, especially of the knee, but clinicians also consider HA for other synovial joints where lubrication has deteriorated. Because HA works by improving the viscoelastic properties of joint fluid, patients often describe easier movement and less “grinding” discomfort during day-to-day tasks. As always, we start with a detailed scan and consultation to confirm the diagnosis and whether HA, a cortisone injection, or another pathway is the more suitable option for your goals and timeframe.

Ostenil Plus has an excellent safety record. Most reactions are short-lived, such as temporary soreness or a feeling of fullness in the joint after the procedure. As with any joint injection, there is a small risk of flare or infection, which we minimise with aseptic technique and imaging guidance. Allergic reactions are uncommon because Ostenil® Plus HA is produced by bacterial fermentation and contains no animal protein; mannitol helps stabilise the HA in the joint.

If you have a history of sensitivities, please raise this at your consultation. You should avoid strenuous activity or heavy exercise for 48 hours after treatment, but can otherwise continue normal daily activities.

Response varies. Some people notice easier movement within a couple of weeks as the joint fluid’s viscosity improves, while others take longer. In clinic experience and manufacturer data suggest symptom relief can persist for months, but results are individual and influenced by overall joint health and activity. NICE notes that evidence for hyaluronan injections across osteoarthritis is mixed overall, which is why we frame HA as one part of a broader plan that may also include exercise therapy, weight optimisation and flare management. Most patients begin to feel relief within a few weeks of treatment, with the effects lasting for several months. The injection can be repeated if necessary, depending on your symptoms and clinical response. To maintain joint health, you may be advised to combine treatment with gentle exercise, physiotherapy, and weight management where appropriate.

No, Ostenil® Plus contains purified hyaluronic acid, not corticosteroid. Steroids reduce inflammation quickly and can be very useful for short-term flare control; hyaluronic acid is a lubricant and shock-absorber that aims to improve the quality of joint movement over a longer window.
