What are major causes of the shoulder pain?
Shoulder pain can be acute or chronic:

What are the common causes of shoulder pain conditions?
- Underlying conditions such as diabetes, rheumatism, gout, or crystal deposition can result in shoulder problems.
- Calcific tendinitis – This is unusual but is a debilitating condition and can trigger shoulder pain.
- Traumatic causes can be acute or insidious. Sports and gym-related injuries are usually of acute onset.
- The shoulder is affected not only by heavy physical work but also by the modern era use of a computer mouse and bad posture, which translates pressure onto the shoulder.
- Subacromial/subdeltoid bursitis – Severe shoulder pain is caused by abnormal fluid accumulation between the deltoid and supraspinatus tendons. Mostly young people suffer from this condition. Bursitis can be idiopathic or maybe with other joint issues.
- Chronic tendon inflammation aggravates and can result in tendon tears, which result in severe shoulder pain.
- In many cases, an arthritic swollen and thickened acromioclavicular joint (a small joint above the shoulder) itself is a cause of shoulder pain. This can exert pressure on the supraspinatus, resulting in impingement and bursal fluid accumulation.
- Fluid accumulation in the tendon sheath of the Biceps tendon can be inflammatory and very painful. It’s called tenosynovitis.
- Minor or major trauma can lead to shoulder musculature, tendon, or ligamentous tears.
- Severe Osteoarthritis – Advancing age is the main causative factor.
- Chronic and severe arthritis can consequentially result in complete tendon tears and fluid accumulation in the joint.
- Frozen Shoulder or Adhesive capsulitis: This debilitating condition commonly causes an extreme reduction of shoulder movements. This usually is the result of chronic shoulder conditions in which the shoulder capsule becomes thickened and results in adhesions.

What Ultrasound guided procedures are available to alleviate shoulder pain?
- Steroid Injections: Steroids have conventionally been used as blind injections to relieve shoulder pain. However, ultrasound guidance is the method of choice in the modern era as doctors can accurately localise a precise point of target while avoiding healthy tissue from needle injury. Most of the patients get immediate pain relief. A local anaesthetic is used before the steroid injection to reduce the injection pain.
- Hyaluronic Acid Injections: Hyaluronic acid is a lubricating fluid which gives a cushioning effect in severe arthritic conditions. Sometimes, it can be combined with steroids for quick pain relief.
- Ultrasound-guided barbotage: Ultrasound guidance is used to break and sometimes withdraw the tendon calcifications, which are a source of tendon inflammation.
- Hydrodilatation/Hydrodistension for frozen shoulder: It is an ultrasound-guided procedure. After local anaesthetic and aseptic conditions, an intra-articular steroid is given, along with a large quantity of normal saline fluid to break the capsular adhesions. This procedure has marvellous results in many cases.
- Suprascapular Nerve Blocks: A suprascapular nerve block is used to alleviate pain due to severe rotator cuff tendon tears, tendon inflammations and severe arthritis resistant to steroid injections. This procedure is performed with ultrasound guidance and under strict aseptic technique. This technique has very promising results.
Any post-procedural advice?
It’s advisable not to drive after the procedure. You are advised not to perform heavy physical activity for 2- 3 days. You need to start physio exercises immediately and increase the frequency of these with the passage of time to achieve the very best results.
