What knee conditions can ultrasound-guided injections treat?

Ultrasound is widely used as one of the best screening tools to evaluate arthritic conditions, tendon problems and traumatic and non-traumatic inflammatory conditions such as tendon inflammation, and tendon or peripheral ligament tears. Ultrasound guidance is used for the precise location of the target point for the injections to be instilled for pain relief.

CAUSES OF KNEE PAIN

Knee pain can arise from many causes, including:

Osteoarthritis

A degenerative joint condition resulting in pain and stiffness. This usually is wear and tear that occurs with age although underlying conditions may exacerbate it.

Bursitis

Bursas are fluid filled structures in various places around the knee. Inflammation of the bursa can result in knee pain and restriction of movement.

Meniscal Injury

Meniscus is a C-shaped cartilage inside the knee. Damage to the meniscus, often occurring from twisting movements. A torn meniscus may cause pain, swelling and or stiffness.

Patella-Femoral Joint Problems

Articular cartilage protects the kneecap and thigh bone from injury. If the cartilage gets damaged our bones are exposed to each other, this can result in pain and damage to the bone. This ultimately results in osteoarthritis. Cartilage can be damaged due to direct impact, trivial trauma or age-related changes. Patella femoral joint problems results in anterior knee pain. Anterior knee pain can also be caused by anatomical variations.

Ligament Injuries

There are two important internal ligaments in the knee; the anterior cruciate ligament (ACL) and the posterior cruciate ligament (PCL). There are further medial and lateral collateral ligaments on both sides of the knee. The ligamentous injuries are commonly seen in trauma settings predominantly in sports.

COMMON KNEE CONDITIONS

Other common knee conditions we come across are:

Baker’s Cyst: A fluid filled sac like swelling at the back of the knee often indicative of underlying knee issues. Bakers’ cyst may restrict flexion movement. There can be severe pain if the Baker’s cyst gets ruptured. Symptomatic Baker’s cysts can be drained by ultrasound guidance in cases of severe symptoms.

Cartilage Tear: Damage to the cartilage, which can occur due to wear and tear or acute injury.

Iliotibial Band Syndrome (ITB): Characterized by pain on the outside of the thigh, often exacerbated by specific activities.

Runner’s Knee: Pain over the front of the knee, typically associated with patello-femoral joint problems.

Pre-patella Bursitis: Inflammation of the bursa due to excessive kneeling. Painful bursal fluid can be drained by ultrasound guidance in certain conditions.

OUR APPROACH TO TREATMENT

We utilize advanced ultrasound techniques to accurately diagnose knee conditions, allowing for precise treatment planning. Our interventions may include Ultrasound-Guided Treatments: Targeted injections and therapies delivered with precision. Local steroids are commonly used to alleviate the knee pain.

In advanced resistive cases in addition to steroids, we may advise and use a non-steroidal solution known as Hyaluronic acid (Ostenil – Ostenil Plus) which has been developed specifically to treat osteoarthritis. This is considered quite effective in the knee by decreasing the pain and stiffness. These injections give a good window into the rehabilitation programs.

Before any injection or procedure our consultant radiologist performs an ultrasound and discusses the findings with you. After explanation of the intended benefits and possible side effects, the radiologist will get your consent before the procedure. You will get a full opportunity to ask any questions.

If you’re suffering from knee pain, our team including expert MSK radiologist is here at Sonorad to help. With a focus on accurate diagnosis and personalized treatment, we aim to alleviate your discomfort and restore your mobility. The booking process was really easy as well. Talked to the lady on the phone who was very attentive and I was able to make the booking online pretty easily.” AJ from Slough

Knee Pain Treatment FAQs

What is ultrasound?

Ultrasound is a fantastic tool that has revolutionised the current Physical and medical practice.

Ultrasound constructs the pictures of the muscles, tendons, ligaments, nerves and joints (small or big ) throughout the body. It helps diagnose sports injuries, sprains, strains, muscle tears, trapped nerves, arthritis and rheumatology conditions.

Ultrasound does not use X radiation and is considered the safest, non-invasive modality for musculoskeletal problems, whether sports/ work or age-related.

What is Ultrasound Imaging of the Musculoskeletal System?

Ultrasound imaging is a non-ionising and non-invasive medical test that helps physicians diagnose and treat medical conditions. It is safe and painless. It produces pictures of the inside of the body using sound waves. Ultrasound imaging is also called sonography. It uses a small probe called a transducer and gel placed directly on the skin. High-frequency sound waves travel from the probe through the gel into the body. The probe collects the sounds that bounce back. A computer uses those sound waves to create an image. Ultrasound exams do not use radiation (No x-rays). Because ultrasound captures images in real time, it can show the structure and motion of the body organs. Thanks to new additions microflow can detect minor inflammations. Ultrasound images of the musculoskeletal system provide pictures of muscles, tendons, ligaments, joints, nerves and soft tissues throughout the body.

What does an ultrasound on knee do?

Ultrasound helps to categorize the degree of arthritis and can check joint fluid quadriceps and patellar tendons. Ultrasound is a guidance to put steroid injections in the right place. Joint fluid can be drained or aspirated through ultrasound guidance. Cysts such as Baker’s cysts release pressure during movements.

Does ultrasound show knee cartilage damage?

Some cartilages can be assessed with ultrasound which gives an idea of the damage. Ultrasound can assess the peripheral menisci. Ultrasound has a limitation to visualise cruciate ligaments.

How do you describe an ultrasound guided injection?

Usually, your doctor requests us to perform an ultrasound-guided injection. If you do not have a referral from your doctor, then please book a consultation with our consultant radiologist to understand your condition.
The consultant radiologist performs an ultrasound of the knee to precisely locate the correct placement of the injection needle. Under strict aseptic conditions, your skin is cleaned and a local anaesthetic is given to temporarily numb your skin and steroid to reduce the pain and inflammation.

Can I walk after a cortisone injection in my knee?
You can walk after injection but try not to do hard work for a couple of days.
Can I drive after ultrasound guided injection?

It’s very unlikely to have a major post-injection side effect.

However, as a precaution, you are advised not to drive back by yourself as skin local anaesthetic may take some time to wear off. Please arrange for a family member, or friend or use taxi/public transport to travel back. Most of the motor insurance companies also advise the same.

What types of ultrasound guided injections can I have in my knee?

Steroids are used to reduce inflammation for rapid pain relief.

Hyaluronic Acid (Ostenil/Ostenil Plus) is a non-steroidal injection and is similar to the joint fluid. This lubricates the joint cavity and relieves pain. Depending upon the condition, the hyaluronic acid injections are used 3 times at two-week intervals for a long-lasting effect. Sometimes steroids and Ostenil can be combined for immediate and long-term relief.

For more information about hyaluronic acid injections please visit our web page Hyaluronic Acid Injections Slough – Highly Experienced Clinician (sonorad.co.uk)

How do I manage bruising to my knee after treatment?

There are usually no significant side effects. But if slight bruising happens, it will ease up in one or two days. Very rarely joint infection can happen but in strict aseptic conditions, this has been very rarely noted. We advise patients to get treatment from a qualified and well-trained health professional.

How many injections will I need?

It’s advisable to have an interval of at least 3 months between two steroid injections. But this all depends on patient to patient.

How long before I feel knee pain relief?

There is a variable response to steroids. Most of the time steroid effect kicks off quickly but in other cases, it may take a few days. Hyaluronic acid injection effects take a little more time to relieve the pain but their effects are considered long-lasting.

What are the advantages of ultrasound guided injections?

There is a significant body of research which demonstrates that ultrasound-guided injections give better outcomes such as improved levels of pain relief, longer-lasting benefits, and improved post-injection function of the injected area, compared with injections that are done without ultrasound guidance (e.g.blind/surface marked injections).

There is also growing evidence that guiding the injection reduces the risk of trauma to local tissues or misplacement of the needle and/or the injection substance. This improves the safety of the injection and reduces the risk of adverse events associated with injection therapy. Because the location of the injection is more precisely known it reduces the chances of having unnecessary injections.

Many patients find ultrasound-guided injections to be more comfortable as often the required depth of the injection can be reduced

Benefits of ultrasound-guided injection

  • Ultrasound scanning is non-invasive and painless (no needles or injections).
  • Ultrasound imaging is extremely safe and does not use X-radiation.
  • Occasionally, an ultrasound exam may be temporarily uncomfortable, but it should not be painful.
  • Ultrasound is widely available, easy to use, and less expensive than most other imaging methods.
  • Ultrasound scanning gives a clear picture of soft tissues that do not show up well on x-ray images.
  • Ultrasound provides real-time imaging. This makes it a good tool for guiding minimally invasive procedures such as needle biopsies, fluid aspiration, and treatment of tendon inflammations.
  • Patients with certain cardiac pacemakers and metallic implants or fragments in the body often cannot be safely exposed to the strong magnetic field of magnetic resonance imaging (MRI); however, patients can safely receive ultrasound imaging.
  • Ultrasound is also an excellent alternative to MRI for claustrophobic patients.
  • Compared to MRI, ultrasound may provide greater internal detail when assessing soft tissue structures such as tendons and nerves.
  • The unique ability to capture real-time images gives ultrasound an edge to see the dynamic movement of tendons, joints or extremities.
  • Ultrasound imaging is faster than MRI and does not require the patient to remain completely still, allowing infants to be imaged without sedation.
