What knee conditions can ultrasound-guided injections treat?

Ultrasound is widely used as one of the best screening tools to evaluate arthritic conditions, tendon problems and traumatic and non-traumatic inflammatory conditions such as tendon inflammation, and tendon or peripheral ligament tears. There are two important internal ligaments in the knee; the anterior cruciate ligament (ACL) and the posterior cruciate ligament (PCL). There are further medial and lateral collateral ligaments on both sides of the knee. The ligamentous injuries are commonly seen in trauma settings predominantly in sports.

COMMON KNEE CONDITIONS

Other common knee conditions we come across are:

Baker’s Cyst: A fluid filled sac like swelling at the back of the knee often indicative of underlying knee issues. Bakers’ cyst may restrict flexion movement. There can be severe pain if the Baker’s cyst gets ruptured. Symptomatic Baker’s cysts can be drained by ultrasound guidance in cases of severe symptoms.

Cartilage Tear: Damage to the cartilage, which can occur due to wear and tear or acute injury.

Iliotibial Band Syndrome (ITB): Characterized by pain on the outside of the thigh, often exacerbated by specific activities.

Runner’s Knee: Pain over the front of the knee, typically associated with patello-femoral joint problems.

Pre-patella Bursitis: Inflammation of the bursa due to excessive kneeling. Painful bursal fluid can be drained by ultrasound guidance in certain conditions.

Ultrasound is a fantastic tool that has revolutionised the current Physical and medical practice.

Ultrasound constructs the pictures of the muscles, tendons, ligaments, nerves and joints (small or big ) throughout the body. It helps diagnose sports injuries, sprains, strains, muscle tears, trapped nerves, arthritis and rheumatology conditions.

Ultrasound does not use X radiation and is considered the safest, non-invasive modality for musculoskeletal problems, whether sports/ work or age-related.

What is Ultrasound Imaging of the Musculoskeletal System?

Ultrasound imaging is a non-ionising and non-invasive medical test that helps physicians diagnose and treat medical conditions. It is safe and painless. It produces pictures of the inside of the body using sound waves. Ultrasound imaging is also called sonography. It uses a small probe called a transducer and gel placed directly on the skin. High-frequency sound waves travel from the probe through the gel into the body. The probe collects the sounds that bounce back. A computer uses those sound waves to create an image. Ultrasound exams do not use radiation (No x-rays). Because ultrasound captures images in real time, it can show the structure and motion of the body organs. The consultant radiologist performs an ultrasound of the knee to precisely locate the correct placement of the injection needle. Under strict a-septic conditions, your skin is cleaded and a local anaesthetic is given to temporarily numb your skin and steroid to reduce the pain and inflammation .

Can I walk after a cortisone injection in my knee?
You can walk after injection but try not to do hard work for couple of days.
Can I drive after ultrasound guided injection?

It’s very unlikely to have a major post injection side effect.

However, as a precaution you are advised not to drive back by yourself as skin local anaesthetic may take some time to wear off. Please arrange some family member, friend or use taxi/ public transport to travel back. Most of the motor insurance companies also advise the same.

What types of ultrasound guided injections can I have in my knee?

Steroids are used to reduce inflammation for rapid pain relief.

Hyaluronic Acid ( Ostenil/Ostenil Plus) is non-steroidal injection and is similar to the joint fluid. This lubricates the joint cavity and relive pain. Depending upon the condition, the hyaluronic acid injections are used 3 times with two weeks interval for a long-lasting effect. How do I manage bruising to my knee after treatment?

There are usually no significant side effects. But if slight bruising happens, it will ease up in one or two days. Very rarely joint infection can happen but in strict aseptic conditions this has been very rarely noted. We advise patients to get treatment from a qualified and well-trained health professional.

How many injections will I need?

Its advisable to have an interval of at least 3 months between two steroid injections. But this all depends patient to patient.

How long before I feel knee pain relief?

There is variable response to steroids. Most of the time Steroid effect kicks off quickly but in other cases, it may take few days. Hyaluronic acid injection effects take a little more time ti relieve the pain but their effects are considered long-lasting.

  • Ultrasound imaging is faster than MRI and does not require the patient to remain completely still, allowing infants to be imaged without sedation.
